If you require a sick note then please call any weekday from 8am for a same day face to face appointment with a Doctor or Advanced Nurse Practitioner. Please note that Sick note/Med3 can only be issued when they are due, although they can be back dated they cannot be future dated.
Extended Sick note/med3 fit note

Please note that this will be forwarded onto the clinician, if the clinician does want to discuss this further then you will be notified by a member of the reception team who will be in touch.
7 days off sick or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification. This can be collected from main reception.
More than 7 days off sick
If you’re off work sick for more than 7 days, your employer will usually ask for a fit note (or Statement of Fitness for Work) from a healthcare professional. Fit notes are sometimes referred to as medical statements or doctor’s notes Med3.
